By opting in, users may receive appointment reminders, general clinic information, mental health tips, and other relevant updates via SMS. Messages are delivered through text messaging to the mobile number provided by the user.
Purpose: The chatbot is designed to provide general information about psychiatric care and mental health. It is not a substitute for professional medical advice, diagnosis, or treatment. If you need professional advice, please contact the clinic where you are being treated to speak to a licensed healthcare provider.
Not a Healthcare Provider: The chatbot is a digital tool and does not constitute a healthcare provider-patient relationship. Its responses are automated and based on a predefined set of data and cannot replace the nuanced advice of a human professional. Users should not interpret the information provided as medical advice or a diagnosis.

Rates Disclosure: Message and data rates may apply to all messages sent and received through the HoloMD SMS program. Please consult your mobile carrier for details regarding your plan and any applicable charges.
Opt-Out Instructions (STOP Keyword): To stop receiving SMS messages from HoloMD, you may reply "STOP" to any message you receive. Upon receipt of your STOP request, you will no longer receive SMS communications from HoloMD, except for a final confirmation message confirming your opt-out.
Help Instructions (HELP Keyword) and Customer Care Contact: If you need assistance or more information about the HoloMD SMS program, reply "HELP" to any message. For further support, you may contact customer care at info@holomd.ai.
Emergency Situations: The chatbot is not designed to handle situations involving self-harm, harm to others, any other form of crisis, or other emergency situations. In case of an emergency, please contact your local emergency services immediately, dial 988, or go to the nearest emergency room.
Carrier Liability Disclosure: Carriers are not liable for delayed or undelivered messages.

Insurance Billing and Copays
HoloMD will bill your insurance provider for the services rendered. I hereby assign all medical and/or mental health benefits to which I am entitled under my health insurance policy for services rendered. It is the patient's responsibility to ensure that their insurance information is accurate and up to date. Any copays, coinsurance, or deductibles as determined by your insurance plan are the patient's responsibility and must be paid at the time of service or upon receipt of an invoice.
Patient Fees and Liability
HoloMD does not forgive patient fees or patient liability owed, except in exceptional cases which will be reviewed on a case-by-case basis. Patients are responsible for any amounts not covered by their insurance, including but not limited to copays, coinsurance, deductibles, and any services not covered by their insurance plan, which may range from $5 to $20 per month while conversing with your doctor's Holo.
Contact Information
Patients are required to keep their contact information, including address, phone number, and email address, up to date with HoloMD. This ensures that billing statements and other important communications related to your use of Dr. Holo are received in a timely manner.
Communication
By enrolling in our SMS program, you agree to receive messages from HoloMD via SMS to the mobile number you provided. Messages may include updates, alerts, reminders, promotional content, and other types of notifications related to our services and offerings. Delivery is via text message (SMS). Message frequency varies. Message and data rates may apply. You may opt out of receiving messages at any time by replying STOP to any message. After you send the SMS message STOP, you will receive a confirmation message and will no longer receive further SMS messages from us. For assistance, reply HELP to any message or contact customer care at +1 234 437 4656. Carriers are not liable for delayed or undelivered messages.
Collection Attempts
HoloMD will make a minimum of three attempts to collect any patient portions owed. These attempts may include phone calls, emails, and mailed statements. If after these attempts the balance remains unpaid, the account may be referred to a collection agency, and additional fees may apply.
